The number of failures and their frequency in the water supply system have a direct impact on the amount of actual water losses. The measure of the failure frequency of the water distribution system is the rate of damage intensity. The failure intensity index can be used for both point and line features. The failure intensity index is also used visually for groups of objects or entire systems. The aim of this article was to check and present the assessment of the failure rate of water distribution systems in administrative units in the Dolnośląskie Voivodeship and the Opolskie Voivodeship in Poland in 2015-2019 based on the calculated values of the failure intensity index. The article presents the results of analyses and the classification of counties of the research area in terms of the failure rate class. The next part of the article presents the results of the failure intensity index for each year separately for the Dolnośląskie and Opolskie Voivodeships. The obtained results were compared with the average value of the failure intensity index found in the literature.
